Healthcare providers, you are the most trusted source of information for your patients’ health decisions. Your strong recommendation to get a COVID-19 vaccine is one of the most important factors in your patients’ decision to accept vaccination.
In order to provide COVID-19 vaccines, you must have a number agreements in place with NMDOH and the CDC. NMDOH has provided this portal to digitally complete, sign, and submit the required documentation.

Sign Up to Become a COVID-19 Provider!
The first step is to request access to the COVID Provider Portal. This portal is intended for providers that are planning on administering the COVID-19 vaccine(s). These providers must have agreements in place with the CDC and the New Mexico Department of Health in order to administer COVID-19 vaccine(s). Those agreements can be completed and submitted via the COVID-19 Vaccine Provider Portal. Click Here to request access to the COVID-19 Vaccine Prover Portal in order to fill out the agreements.

Order Printed Copies at No Cost
Do you want brochures, buttons, mailers, or posters for your practice. You can submit your order through the training module and the State of New Mexico will print and ship the materials to you at no cost.
Take Care New Mexico Provider Certificate
The Take Care New Mexico Provider Certificate is an online, on demand video training on COVID-19 vaccine administration for doctors and their staff. Your team will learn how become a provider, prepare your office, ordering and supply management, required reporting requirements, and more. Administering the COVID-19 vaccine is complex, but the New Mexico Department of Health developed a comprehensive training program to ensure ease of compliance and access to all New Mexicans.
The training takes approximately 2 hours and upon completion, providers receive a certificate, toolkits and ongoing support. In addition, New Mexico Department of Health will align consumer marketing efforts to encourage New Mexicans to make appointments with participating providers through the online portal.
